The growing number of chronic diseases in the adult population highlights the importance of health care and disease prevention, as these diseases account for as much as 80% of all deaths. Chronic diseases are more common in older people and contribute to poorer quality of life and frailty. Despite established clinical guidelines, the care of chronic patients does not achieve the desired results, as only a small proportion of patients have controlled blood pressure or blood sugar. It is essential to actively involve patients in their treatment and to provide person-centred care based on respect for patients as individuals and on building trust between them and healthcare professionals. Research on person-centred care interventions shows positive effects on behavioural change and higher quality of life in patients with chronic diseases. 

The project focuses on examining the links between a sense of coherence, frailty, life management strategies, and quality of life in adults with chronic diseases. The main objectives are to validate research instruments and to investigate quality of life and the relationship between various factors and quality of life. We will conduct a cross-sectional study using standardized questionnaires and analyze the data using various statistical methods. The expected results include the development and shortening of the Slovenian version of the questionnaires and the preparation of educational interventions.
